This is an excellent opportunity to receive service hours!
This agreement releases Oregon Area Chamber of Commerce, and all participating volunteer organizations and persons from all liability relating to injuries that may occur during the Oregon Community Cleanup Project on April 24th or May 1 and any and all visits to the locations I am volunteering at. By signing this agreement, I agree to hold The Oregon Area Chamber of Commerce and all participating organizations entirely free from any liability, including financial responsibility for injuries incurred, regardless of whether injuries are caused by negligence. I also acknowledge the risks involved in The Oregon Community Clean Up Project. These include but are not limited to mowing, brush removal, painting, picking up rubbish, and other physical activities throughout the Oregon Area. I swear that I am participating voluntarily, and that all risks have been made clear to me. Additionally, I do not have any conditions that will increase my likelihood of experiencing injuries while engaging in this activity. By signing below I forfeit all right to bring a suit against The Oregon Area Chamber of Commerce for any reason. In return, I will receive the satisfaction of helping my community. I will also make every effort to obey safety precautions as listed in writing and as explained to me verbally. I will ask for clarification when needed. If under 18 this form must be filled out by a parent or guardian.
